You can exercise, eat healthily, and follow dietary plans: your legs just won’t lose circumference or might even grow wider? Even mild movements hurt, you bruise easily, and you can’t stand for long without feeling exhausted and tense? Then you likely suffer from lipedema, just like many other women – a fat distribution disorder. Legs, hips, and even arms can be affected, often severely restricting quality of life. Unfortunately, lipedema is not curable.
To date, the causes of lipedema legs are still largely unexplored. Unfortunately, lipedema is still considered incurable. The so-called thick leg disease is a chronic adipose tissue disorder that primarily affects women. It is believed to have a genetic predisposition, but hormonal factors also clearly contribute to lipedema legs. The good news is: being incurable does not mean you can do nothing to relieve your symptoms and slow the progression of the disease. The earlier you start countermeasures and the more consistently you stick with them, the more symptom-free you can be.

Recognizing lipedema legs: these are the typical symptoms
Lipedema is very often confused with overweight or obesity, even though these are completely different conditions. With classic overweight, the body usually gains circumference evenly. The typical dimples on the legs are usually cellulite, which is painless. In contrast, you can recognize lipedema legs by the following symptoms:
- Symmetrical increase in both legs: If lipedema is present, the volume in both legs increases evenly. The swelling itself is usually asymmetrical. The legs overall appear disproportionate.
- Pressure-sensitive tissue: The affected adipose tissue is very sensitive. Even light pressure leads to unusual pain. This pain occurs because the enlarged fat cells press on nerve pathways.
- Large calves and feet compared to the torso: Very typical of lipedema legs is that they appear unusually large compared to the torso. This is referred to as "rider’s pants" appearance.
- Cellulite-like dimples: As the disease progresses, pronounced pits and dimples form in the adipose tissue. However, these are much more pronounced than with regular cellulite.
- Prone to bruising: The skin on lipedema legs is usually more sensitive and prone to bruising, even with only minor injuries.
- No improvement through diet or exercise: Unlike weight gain from poor diet or lack of exercise, lipedema symptoms often do not improve through diet or exercise alone.
- Heaviness and fatigue: Those affected usually feel a constant heaviness in their legs. They cannot stand for long periods or sit for extended periods.
Often, the same signs of lipedema legs also appear in the arms. The fat distribution disorder can affect the entire body – even the knees, hands, and feet.
Typical distribution patterns for lipedema legs
Unlike overweight, fat is not distributed evenly in lipedema legs, but instead follows typical distribution patterns. The so-called thigh type only affects the hips and thighs. The pathologically increased adipose tissue forms the so-called rider’s pants. There is also the calf type, in which the calves are additionally affected by lipedema alongside the thighs. In severe cases, even the ankles can swell. A clear step with a sort of fat collar at the transition to the ankles is characteristic in this case.
There are different stages of lipedema in the legs
Lipedema in the legs is divided into three distinct stages.

Stage 1: Asymmetrical fat distribution in the legs
In the first stage, those affected often don’t even realize they have an illness. There is a uniform thickening of the lower skin layers. The thighs already show a tendency toward rider’s pants, but the skin is still relatively smooth. -
Stage 2: Dimples in the skin’s surface
In this stage, clearly thickened and nodular skin areas are visible. These can even be the size of apples. The torso and legs are disproportionate to each other. The fat deposits in the thigh area are no longer proportional to the torso. -
Stage 3: Pronounced volume increase
This stage is characterized by a significant increase in volume in the upper and sometimes lower legs. The tissue is hardened. This development usually also impairs gait, and those affected develop bowed legs (X-shaped legs).
Incidentally, the severity of symptoms does not correlate with the stage of the disease. Severe pain can occur as early as the first stage.
How the lipedema pinch test works
Legs and arms are affected by the fat distribution disorder about equally often. Therefore, the pinch test is also performed in these body areas. You can check if your symptoms are due to lipedema by gently pinching the skin. A slight fold is all that is needed – you don’t need to apply much pressure. If you already feel excessive pain or pressure, this is a clear sign of a fat distribution disorder. Of course, this test is only for orientation and does not replace a visit to a doctor.
What can you do about lipedema legs?
For affected women, the uncontrolled fat buildup in the legs is a significant burden. Daily tasks like putting on and taking off clothes, walking, or waiting in line become a real challenge. The motivation to confront the illness and do something about the painful swelling is all the greater.
Exercise as part of lipedema leg therapy
Movement and exercise should always be a fixed part of your daily routine if you suffer from lipedema legs. Physical activity can positively influence the course of the disease and relieve symptoms. You counter the adipose tissue with new muscle mass, improve your circulation, and stimulate lymph flow.
The following exercises and sports are particularly suitable for lipedema legs:
- Brisk walking or Nordic walking
- Cycling outdoors or on a stationary bike
- Swimming and aqua training
- Strength training to build leg muscle
- EMS (electrical muscle stimulation)
- Yoga and Pilates
However, you should avoid high-intensity training that involves jumping, hopping, or quick direction changes.
Proper nutrition for lipedema
A balanced diet strengthens your immune system and supports your metabolism. This is particularly important for lipedema patients. A low-calorie, low-fat diet and sufficient fluid intake throughout the day are beneficial. Also pay attention to regular meals to keep your blood sugar stable throughout the day. The goal of good nutrition for lipedema legs is always to avoid overweight, as this places additional strain on the tissue and worsens symptoms.
Suitable foods to support lipedema treatment:
- Fruits and vegetables (especially broccoli, spinach, carrots)
- Fish (salmon, herring, and mackerel)
- Dairy products
- Legumes (e.g., peas and beans)
- Orange juice
Even though it can sometimes be difficult: you should reduce sweets if possible, as well as sodas, refined grain products, and fried foods.
Liposuction: Lipedema leg treatment through surgery
Conservative treatment for advanced lipedema is liposuction. Good results can be achieved with surgical intervention even in early stages. The goal of the surgery is a significant reduction of adipose tissue and thus also leg circumference. This restores better mobility and can significantly reduce pain. You should continue to wear compression garments even after successful surgery. Tightening lipedema legs: what happens during surgery
During liposuction, pathologically increased adipose tissue is suctioned out under general anesthesia. Up to eight liters can be removed in total. There are different methods for the procedure. Usually, a special tumescent solution is injected into the affected areas on the leg. A vibrating cannula then loosens and dislodges the fat cells, which can then be suctioned out. After the procedure, patients must wear special compression pants so that the legs regain their proper shape.
What many people don’t know: Fat cells that have been suctioned out cannot grow back. The number of fat cells in our body is genetically determined. If they are removed during liposuction, they are gone permanently. Surgery is thus a sustainable method for treating lipedema in the legs. However, it can only provide long-term relief in combination with other measures such as regular manual lymphatic drainage and wearing compression garments.
Lipedema leg surgery: Health insurance pays under these conditions
Under certain conditions, health insurance covers the costs of lipedema treatment if it is not a purely cosmetic procedure. It is important that you consult a licensed contracted doctor or a hospital listed with your health insurance company. Cost coverage is still very strictly regulated to date and remains an individual case decision. If you have been diagnosed with stage 3 lipedema legs, you will in any case have good chances that your insurance will cover the surgery costs.
Manual lymphatic drainage & decongestion therapy for lipedema
An essential part of treating lipedema in the legs is manual lymphatic drainage. This is a gentle, targeted massage where the therapist applies even pressure to the swollen tissue. The goal of this massage is to stimulate lymph flow so that toxins, fats, and excess proteins are eliminated from the body more quickly.
Lymphatic drainage also increases the capacity of the entire lymphatic system, so fluid can no longer accumulate in the tissue to the same extent. The connective tissue is strengthened long-term, and pain relief is achieved. Lymphatic drainage is usually combined with a compression measure.
This is why compression is so important for lipedema legs
The typical lipedema symptoms are caused by a buildup of fluid in the adipose tissue. Compression garments apply even pressure to the affected areas, which helps reduce fluid buildup and swelling. It also improves blood circulation and removes toxins from the legs faster and more effectively. Compression pants stop further expansion of fat cells. This can prevent the disease from progressing.
Traditional compression garments have their drawbacks
When people think of compression garments, they usually feel uncomfortable. Compression stockings or pants are usually very difficult to pull over already swollen legs – often impossible without help. Additionally, people easily sweat under the tight clothing, and it just feels uncomfortable. Compression garments rarely meet aesthetic standards – they are what they are: functional.
